Medical Model Skilled Nursing
Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) Registered nurses provide a broad range of nursing services based on a thorough assessment of the client’s health care needs and development of an individualized care plan. Examples of services provided include wound care, medication management, disease management education, infusion nursing, equipment management and care management. Physician orders and efficient care coordination are integral aspects of nursing services. Working in conjunction with the Registered Nurse, Licensed Practical Nurses assist with rehabilitation, dressing changes, disease management teaching and medication management and teaching.
Physical Therapists manage and provide skilled physical therapy to patient/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician’s plan of treatment, considering individual patients’ special or age-related needs. Examples of services include post-surgery rehabilitation, including post orthopedic procedures, decline in condition, fall prevention and improving or maintaining functional level of care.
Occupational Therapists perform therapeutic exercises for fine motor skills and instruct in activities of daily living such as dressing, bathing, eating, energy conservation techniques and caregiver education. Occupational Therapists also address equipment needs such as grab bars, shower chairs and other adaptive equipment. Promoting client independence is always our goal.
Speech / Language Therapists provide treatment to improve speech, swallowing, communication and problem-solving skills. Our Speech therapists are also equipped to provide vital stim treatment for Parkinson’s, neuromuscular disease and dysphagia treatment. Often applied with other therapies, speech therapy significantly contributes to stabilization and recovery.
Medical Social Services
The medical social worker will assess the psychosocial functioning, environmental and the support needs of patients and families. This may include connecting patients and families to necessary resources and support in the community, supportive counseling or helping to expand and strengthen the network of social support.
Infusion Therapy For patients who are no longer treatable with medication taken orally, we offer Infusion Therapy in a home setting. Drugs administered intravenously need the expertise of our staff to properly execute. This type of therapy can be used in various cases including cancer medication, antivirals, antibiotics, pain management and hydration for post-surgical patients.
Medical Home Health Aids
Home Health Aides provide personal care with activities such as bathing, dressing and other activities of daily living during the duration of a medical home health care period. Non-medical model All clients receive a nursing evaluation at the start of care and every 60 days to set up the care plan and assess patient needs. Every month the nurse makes a supervisory visit to ensure that all needs and care plan are being met. All nursing visits are provided free of charge.
Home Health Aides
Home Health Aides Home Health Aides provide personal care to clients who need assistance at home. This usually covers activities of daily living, mobility and meal preparation. Home Health Aides can also be assigned for Respite Care assignments to temporarily take over family caregiver duties.
Our team includes companions and homemakers who contribute to quality living for patients who are home-bound. Companions not only provide companionship, they also supervise clients at home while their family members are out to work or in school for the day. Homemakers, on the other hand, handle household chores and home management.
Orientation and Training
Orientation process includes one on training and precepted visits to assess competency of our employees. Staff are required to attend mandatory in-services throughout the year. This allows us to make sure that staff are aware of new regulations, guidance on processes and has the most up to date information regarding new treatments, modalities and ways to provide care.

We also screen through OIG, SAM and Medicheck databases upon hire and monthly thereafter to ensure that no employees is on any of these lists. Drug testing is performed on random basis.
